Storage of information
Your data will be stored electronically on our computer and will be password encrypted. At intake stage each client will be allocated a code, and this will be used in all session notes, hence making the information in those notes anonymous. As our psychotherapy service is for people over 18 years of age, client files will be kept for a period of 7 years as is required practice. Any handwritten notes generated say during intake or during sessions will be immediately transferred to a format to be stored electronically and the hard copy will be shredded.
Sharing of information
A times it may be necessary to share your information with next of kin or other health care professionals with the intention of providing a comprehensive and safe level of care for you. Your consent will always be sought in advance except for the following circumstances:
- Where we believe that there is a serious risk of harm to you or others
- When we receive information about past or present accounts of abuse or neglect we are mandated to report our concerns to TUSLA and the Garda Siochana if necessary.
- When we are made aware of serious acts of criminality.
As part of professional practice requirements, I attend regular clinical supervision. During supervision aspects of client work may be discussed to support a good level of effective practice; however, all identifying information is not shared and your identity remains confidential. Also, please note that I do not provide court reports.
